I find it hard to answer your second question properly. If it makes sense, I would do anything to have my best clutch hitter #5 or #6 (or #4 if he's one of my best hitter), and I would do my best to have my worst clutch hitter #2 or #3. But I'm not sure I could put up numbers on the thresholds that would translate into "it does not make sense anymore".
I guess it depends on the ballpark too. In a homerun stadium, I would go with the homeruns rather than the clutch in my decision-making.

No, you first decide who are your best 9 players. Then, you decide of the line-up.
