Injury rule clarification ?

Can someone clarify the injury rule? I read this injury info that someone posted:
* Players with over 599 Strat plate appearences (defined as at bats + walks, not actual MLB plate appearences) can only be injured a max of 3 games.
Players over 680 pa's can not be hurt.
Does the the 3 game max mean per injury roll or 3 games max per season
for a player with 649 plate appearances? Since I have a 600 + player who
is now on his second 3 game stint, I would assume that it's per roll but just
want to get a rule clarification from one of you veterans and make sure it's not a glitch.
