How do pitchers get injured while pitching?

I can understand how pitchers get injured while batting, just roll a 2-2. But how do they get injured while they are pitching?

In a DH league, pitchers can get hurt when pitching to the opposing DH. A roll of 6-12 will do it.

Or in real life a line drive off their head or something pop in their pitching arm or ......
