how does this happen?

Remember the Cubs and Moises Alou trying to catch that ball in LF a few years ago. Bart Ball.

I have seen this as well as catcher's interference and out on appeal for other odd occurances.

The advanced SOM boardgame version has a provision for "rare plays". I'm sure that this is incorporated into the SOM-ATG game.
