George Foster triple play

Just curious if anyone knows why this happened. I have George Foster and he got injured on a play for 3 games. He has 4 of his 5 years where his injury is a LO max. On the play he got injured, there were 0 outs and 2 runners on base, and it was only a double play. Why would LO max not be a triple play if it is possible? Not complaining, I prefer to only get the double against me, but curious why this happened, anyone know?

I remember this happening to me with some player, and the explanation was along the lines that with a LO-MAX with 2 players on and no one out, there is an additional split drawn (or 20-sided dice roll). If the split is 1-10 then it's a triple play, otherwise a double play. Someone please correct me if I'm wrong!

I think it's 1-7 = triple play, 8-20 = double play.
