What's up with relief pitcher usage?

I have my setting on aggressive and I'm still having 6 inning starters throw 137 pitchers and spot lefty relievers going 3 innings and getting kicked??

You can assist your aggressive RP settings but also using the "quick hook" and "slow hook" settings as well as the "avoid LH or RH batters" on the individual cards.
