This boxscore shows a player already in the game at a fielding position switching positions to DH in the middle of the game, while the DH was replaced with a position player.

Now in real baseball, if the player in the batting position of the DH becomes a fielder, the DH is lost and the pitcher must be put into the batting order. Right?

In effect, I believe you are correct. If a designated hitter substitutes for a position player, then the DH is forfeited for the remainder of the game. However, in this case, that didn't happen. In this case, a position player was actually moved to DH. While there doesn't appear to be a rule that explicitly prohibits this, it is in effect prohibited by this clause:

Since the DH should have been "locked" into the leadoff position, it would be impossible to substitute someone who was already in the lineup at a different position.

Good catch! Probably a programming challenge to prevent this in a DH league, however.

Interesting. This definitely would be something that was caused by my defensive replacement work, and I apologize. However, it seems this is an extremely rare case, and that's what caused the problem - 2 players getting injured in the same inning, one of them being the DH. I otherwise made very sure that in 99+% of cases, the DH cannot be subbed into like that.
