defensive swaps

Say that I have Granderson in CF and Kemp in RF, but I want to move Kemp to CF late in the game. Do I list Kemp as my first defensive replacement so that he moves to CF and Francouer can play RF, or do I need to list Francouer first in RF and then list Kemp as the second replacement in CF?

Francoeur goes first. Whomever is coming off the bench will always be listed first as below:

Francoeur RF
Kemp CF
