Jamie Carroll (second baseman for visiting team) mysteriously throws out the batter as the walk-off run scores:

*** BOTTOM OF INNING 13 ***
B.Upton 5-7 Single (2B) b-1
E.Longoria 3-6 Strike Out b-0
N.Markakis 6-8 Single (LF) 1-3 b-1
A.Escobar 1-9 Ground Out (2B) 3-H 1-2 b-0

Maybe Jamey Carroll didn't "look the runner back" at 3B well enough, and Upton took off as soon as Carroll let go of the ball. By then, the 1B can't make the throw home in time.

Or maybe a GB(C) and the infield was not in. If the infield is in on a GB(C), the lead runner is out.
