He was F0 because he had not rested enough--there probably were no pitchers left that wouldn't have been F0 after the whole bullpen was used.
don't use the max 1 or 2 innings would be my advice, some of these settings are forced AI settings--that may be one of them (I'm not sure--I don't use that setting, but I know max 6 or 7 inning starts are, so it would make since that max relief innings are too) It may have forced relievers out of the game before they tired.
