SOM does not document this well, if at all. However, I believe if an opposing pitcher pitches more than 1 inning two games in a row, he cannot pitch the next game. However, if he is forced to pitch due extra innings or lots of pitcher changes, he will come in fatigued at F0.
Your opposing pitcher could pitch three games in a row because he only pitched 1 inning in the middle game. My guess is he can't pitch four games in a row regardless, but I really don't know for sure.
And yes an R3 pitcher can pitch 5 innings as long as he is doing well. If you start getting hits and walks against him, his fatigue level will drop and HAL will replace him.
