I'll take a stab.
1. Subs may replace an injured starter or play in different packages (5 or 6 DBs for example).
2. The Q or D starter may or may not be active. You won't know until after the game is played.
3. No penalty for starting a Q or D. Appear to perform at full strength.
4. Not possible.
