I've had a starter go down a few times in the last few years. I generally take the initiative and remove the relief pitcher from the pitching roles. In my experience, when I remove the RP from the pitching roles, HAL generally doesn't use him as an RP in the immediately preceding games.
Another question is begged - "How would I know which RP HAL intends to start?" - Of course, if the injured SPs spot is in the next 3 games, it will show up on the front office page. I don't know this for sure, but I wonder if it might also show up on the "team/schedule" page, where it lists the upcoming starters (according to HAL anyway).

I don't think anyone has seen this expressed as a "rule", but in the experience of various online strat gamers, I don't think we have ever seen a SP* pitcher get injured for more than 3 games. So, the consensus answer on this question is that it appears that SP* injuries are limited to 3 games. This appears to be another benefit of having a SP* pitcher.
One more thing - if a non-SP starter goes down to injury and misses more than one start, HAL will generally use the same RP to replace the SP in each of the required starts. HAL generally uses an RP that has a higher endurance rating, and generally avoids using a closer RP. However, other people might have found exceptions to these comments, maybe. When this has happened to me, I generally change my pitcher preferences to get the other starting pitchers to go longer/deeper into games, in order to avoid a situation where the bullpen is super fatigued, running on F0 for innings at a time.
