I have a player marked "Do not pinch hit for" both RH and LH. He was pinch hit for without injury. WTH?

In the play-by-play, does the pinch-hitter have "role: platoon PH" next to his name? That's the only thing I can think of. I believe "Platoon PH" always overrides the "Do not pinch hit for" setting. Easy to fix in your default lineups.
