Pitchers back then threw underhand, with pitch location the biggest factor (batters could call for high/low pitches, many called balls for a walk, etc). Things changed when they went to overhand pitching, size equating to speed. Candy Cummings was 5'9" and just 120 lbs, and supposedly came up with the curveball. Hoss Radbourne is regarded as a workhorse type, with all those innings in such a short career, but he was just 5'9", 168 lbs.
