Here is a game where Terry Steinbach gets injured in the 9th inning after replacing Ted Simmons in the 7th inning, while no other legitimate catcher was on my team.

yep. that CAN happen. what can't happen is that both get hurt. If Simmons is replaced by Steinbach, then Steinbach can get hurt. Rare, but I'm pretty sure it's happened before.
