Injury Read on Ground Ball

Ozzie Smith gets injured on a GB with men on 2nd and 3rd and 0 outs.
Smith is out and Runner on 3rd scores and runner on 2nd advances to 3rd.
Is his injury a gb B+injury, gb B+ +injury, or a gb A+ +injury?
I assume a gb A+ +injury would have resulted in a DP? or, do DP's only occur with a man on 1st?

Please someone correct me if I'm wrong, but I believe that it could be any of those with men on 2nd & 3rd and the infield back. I think that is the case with gbBs too.

DPs on gbA should only occur with a runner on first.
