From DH to defense?

This is somewhat related to "17 players at .500+ slugging". I was off on two minor details and a second look made me aware of this astonishing event. I thought this was NOT supposed to happen. It was Hornsby who was injured in the third inning with Scales replacing him at second. Then Ortiz, the DH got injured in the 6th without reaching base. When his turn at bat came up with 2 outs in the 7th, HAL pinch hit Jeter who struck out (ruining my all .500 slugging goal). No moves were announced by HAL for the bottom of the 7h. After we hit in the 8th, and with a lead, HAL announced defensive moves, with Jeter, who at this point was the DH placed at ss. Arky was not injured, so there was no urgent need for a ss. I wish we could have rallied more in the 9th to see what would have happened when Scales' turn came up! (He was the player removed when HAL put Jeter at ss, moving Arky to 2nd. (I had not removed the "remove for defense marking for Scales"
AND I also noticed that when Ortiz was injured on a 1-12 roll (lo max + inj) he was given a dp even though there were 2 runners on (1st and 3rd) with no outs. The inning continued. Shouldn't that have been a Triple Play? Does anyone have an explanation for this?
