2 leagues I'm in have a pitcher injured yet advanced injuries is off, what am I missing?

Pitchers can get injured when the DH rolls a 6-12, subject to normal injury chart.
Pitchers at 200.1 innings or above can only be hurt 3 games max. I think there is a no injury at a certain point too (maybe 270.1 or 300+) but it really only applies to ATG now--and I play that very little. In noDH leagues the injuries come on the pitchers hitting card.
