This could be a result of you not understanding the rules of the game. If you use an aggressive relief strategy or put lots of quick hooks or other limitations on your relievers it will not always work out as you want it to. Even if you don't use these limitations you could run into a few games in a row where your SP is yanked early and your bullpen is used and abused. When you have several RPs that have to sit out a game for too many games used in a row your limitations get overruled if you don't have enough RPs in your bullpen.
Also all 1 injury hitters are not created equal. A 1 injury hitter that has less than 600 PA (calculated as AB + BB in this game) could go down for up to 15 games per injury. If you invest in more hitters that have 600+ PA you will limit your injuries to 3 games max or even only to "rest of game" if you have a player with 680 PA. If you use 680 PA guys or even player with a 0 injury rating you will have less injuries but you will also pay more for those players because of it just like anything else in this game. Want all 1's on defense? You pay up for that for instance.
