The reality is that injuries are underdone in SOM and in any simulation. Why? Because you don’t want to deal with your ace pitcher missing the rest of the season with Tommy John surgery. You don’t want to deal with your superstar breaking his arm in the all star game and missing months of time. We want to see our best players play so we play them daily and the risk of injury is far less than realism would require.
There are some who say the injuries are already built into the players cards and to an extent that’s true. Mantle cards lose speed after his knee injuries. So a career altering injury shows in future cards and a career ending injury just means no future cards but really, if we want realism injuries should be possible for the entire season and should be far more frequent in pitching than it currently is.
Of course then we could get into the possibilities of an IR in SOM for longer stretches and can you replace a player and if so how much of the current players salary (if any) do you have available to do so. But that isn’t going to happen
