What do those mean? Is there a legend somewhere?

I remember Bernie describing them at one time away back, but haven't seen anything since?

Boldface 1 means in-game only. Italic 1 means three games. 0 means bullet proof. Regular 1 means 15 games.

They should add an explanation somewhere; I couldn't find one either.

But why would there be a 15 game injury with a 1? Why wouldn't they just classify the prone-ness to injury as a 2?

Because a 2 means the injury comes up on an 11 roll, rather than 12. The player is more likely to be injured with a 2. Like the plain 1, the max is 15 games.

You'll notice that some players have even worse injuries -- Tommie Davis, Danny Cater, Joe Adcock, Willard Hershberger come to mind. Hershberger's injury is a 6. The guy is likely to be injured half the season.

The number equals the total chances on a player's card.
Each card has 108 dice roll possibilities. So, any card with a 1 inj rating has 1 chance out of the 108 possiblities of an injury occurring. The possible placements of the injury on a card with a 1 inj rating are: 1-2, 2-2, 3-2, 1-12, 2-12, 3-12.

Players with fewer than 600 PA from that carded season are eligible for up to 15 games missed due to one injury.
600 to 679 PA = 3-game inj risk
680 PA = in-game inj only

Some players have a 0 rating, which means that they cannot be removed from a game due to an injury.

For example, Carl Yastrzemski's 1967 card has HBP + injury at 2-12 (on each side). The 2-12 has only one chance of rolling per 108 chances on a player's card (standard probability). Because Yaz had 670 PA, he is only a 3-game risk.

No.
On two dice, an 11 has two possibilities: 5-6 and 6-5. A twelve has only one possibility: 6-6.

To the other end, a 7 on two dice can occur six different ways: 1-6, 2-5, 3-4, 4-3, 5-2, 6-1.
