Somewhere I remember reading that the max number of games injured that a player with 600+ AB was 3. Can someone provide the complete AB/Inj max games table?

Very simple:

600+ Plate Appearances (SOM calculates it as AB + BB) = 3 game max injury
599 or less = up to 15 games

FYI:

The 15-game maximum involves possible injuries from "none" to "remainder of game" to various numbers from 1 to 15. The numbers above 3 are changed to 3 for max-3-game players.

On average in a 162-game season, each injury for less-than-600 AB+W players is 3.454 games. For more-than-599 AB+W players, the average injury is 1.829 games. (Both are "not counting the remainder of the game he got injured.")

These numbers account for the fact that end-of-season injuries are shortened if the season ends before the injury "runs out," thus lowering the overall average.

The above is statistically accurate no matter what other variables are factored in -- they're based entirely on the injury tables.

What about pitchers? How is that decided? I just had Killian go down for 15 games, never saw that before.

An * starting pitcher with 200 or more inning pitched can only get injured for a max of 3 games so they never miss a start.

Any other pitcher can get injured for the 15 games max.
